This portfolio focuses on decreasing financial barriers, increasing provider availability and enhancing appreciation of the benefits of care, particularly preventive care. Funded projects strengthen the safety net by serving uninsured children, build the capacity of care providers to meet the needs of underserved children and overcome barriers to care through community outreach to underserved families. Children's Health and Development Program – Phase III

Provide a community-based pediatric multi-subspecialty clinic in Wake County.

Purpose: Capacity issues at the North Carolina Children's Hospital outpatient specialty clinic contribute to increased emergency room use, transfers for inpatient care, and long delays in outpatient appointments. Children with complex and chronic illness have increasing difficulty accessing diagnostic and management services.

Teen Parent Connection

YWCA of the Greater Triangle, Inc.
This program will provide comprehensive community-based support services that focus on reducing disparities in birth outcomes, enhancing parent-child interactions and relationships, and improving educational outcomes for pregnant and parenting teens.

Expand the Summer Food Service Program for children who participate in free and reduced lunch programs during the school year.

  1. Hire a Summer Food Service Coordinator to carry out community education with participating sites, site enrollment, administration and education.

Improve agency infrastructure and strengthen capacity to manage care for adolescents.

Purpose: Teens presenting to Wake Teen often have several challenging health issues compounded by other social/life situation factors that interfere with effective care for health concerns.

  • Strengthen Wake Teen's capacity to manage care for this at risk population by improving agency infrastructure.

How:

  1. Add one full time nurse;
  2. Develop tracking and referral protocols.

SouthLight's Integration Project (SIP): Electronic Health Management

The purpose of this proposal is to build the capacity of SouthLight, Inc. by transitioning to an electronic health management system.

Need: Internal quality improvement assessment indicated a number of deficiencies in regards to how client information is collected and shared: 1) checks and balances for billing; 2) follow-up data for state required information; 3) systematic coding of identification for clients; 4) exchange of information within systems for billing, progress notes, follow-up and success of treatment; 5) continuous QI data; and 6) notifications or reminders for counselors.

Establish a clinic to provide medical services for children living in the Raleigh Rescue Mission and Salvation Army shelters.

Purpose: Equip and staff a clinic to provide medical services for children living in the Mission and Salvation Army.

How:

  1. Provide prompt, consistent care to children entering either shelter.
  2. Determine eligibility for public health insurance or other health access programs.
  3. Work with mothers of children to use health services.

El Pueblo
Continue developing LÌderes de Salud, which connects the growing population of low income, underinsured residents to health services with the assistance of trained lay health advisors.

Purpose: Recent Latino immigrants still adapting to a new language and a new system of care struggle to understand and access existing services while the existing safety net system struggles to provide appropriate care.

El Pueblo
Promote improved health among Latino children through health advisors who work with families, community-based organizations and healthcare organizations. 

What: A project to develop promotores (lay health advisors) to work with community-based organizations in Wake County.

From: El Pueblo - a statewide organization, based in Raleigh, dedicated to strengthening the Latino community; active in health education and training, health screenings and publications.
