This portfolio focuses on decreasing financial barriers, increasing provider availability and enhancing appreciation of the benefits of care, particularly preventive care. Funded projects strengthen the safety net by serving uninsured children, build the capacity of care providers to meet the needs of underserved children and overcome barriers to care through community outreach to underserved families. Interconception Care for Mothers: An Innovative Approach to Reducing Maternal and Infant Mortality and Morbidity

UNC-CH Center for Maternal and Infant Health
The goal of this program is to improve the health of women and infants in Wake County by addressing the interconceptional health needs of low-income mothers.

Need: Infant mortality is the leading cause of child death in NC. In Wake County, 90 infants died before reaching their first birthday in 2006. One out of every 7 babies is born preterm in NC.

Increasing Access for Children Survivors of Domestic Violence

Wake Health Services, Inc.
The Oberlin Outreach Center at the new Interact facility will provide basic primary care services to approximately 250 children and 100 adults, and act as a bridge to other Wake Health Services sites as a permanent medical home for both the parent and children.

Need:
Domestic violence has negative impact on the development, behavior, education, health and mental health of children and youth. The emotional consequences of being injured, harassed, or terrified can lead to an abused parent being less attuned to their children's needs. Safety concerns serve as a barrier for abused parents and children to access health care services.

Increase availability and quality of after-school and summertime community recreational opportunities for school-age youth with cognitive, behavioral, emotional and physical disabilities.

  • Increase participation of children with disabilities in day cares, after school programs and summer camps with typically developing peers.

Increase health care access for children in public housing.

What: Program to integrate family, health and education services to neighborhood families.

Purpose: Create a public housing site program to increase the access of resident children to needed health care that can be sustained by parents who have received training and support regarding enrollment and renewal of health insurance and well visit participation.

Faith Community Support Circles for Homeless Families

Catholic Charities of the Diocese of Raleigh, Inc.
The Support Circle program proposes to improve behavioral, educational, and health outcomes and access to health care for children experiencing homelessness.

Need: An estimated 900 children experienced homelessness in Wake County in 2008. Research demonstrates that homeless children are less likely to have obtained preventive health care services, more likely to be exposed to substance abuse, more likely to develop acute or chronic illnesses, and more likely to use emergency rooms.

Purpose: The Support Circle program proposes to improve behavioral, educational, and health outcomes and access to health care for children experiencing homelessness.

Expansion Planning Grant*

Hire health care consultant to conduct expansion plan and provide guidance to Wake Teen Board of Directors on location, number of clinic sites and collaboration.

Provide comprehensive family-based intervention for children most at risk of poor health and school failure. Bridge gaps in health delivery system in underserved neighborhoods.

Purpose: Bridge gaps in health delivery system with two nurse practitioners and four community lay health advisors working with established contacts in seven underserved neighborhoods.

Educate residents how to advocate for Pediatric Continuity Clinic patients and improve and assure patient access to community services.

Purpose: Improve and assure access to community services for patients at the UNC Pediatric Continuity Clinic with special needs; and educate pediatric residents how to advocate for and secure available community services for patients from health departments, schools, CDSA (Children's Developmental Services Agencies and DSS (Department of Social Services).

Enroll eligible uninsured children in Medicaid and N.C. Health Choice, bring new patients into WHSI practices and increase preventive care visits.

What: Improve enrollment, access, and outcomes for Wake County children.

Purpose: Increase enrollment of Wake County children age birth through nineteen eligible for Medicaid and NC Health Choice and to assist enrolled children in accessing public and private practices accepting these insurances.
