Computer database to support safety net system.

From: Capital Area Healthcare Collaborative (CAHAC) - a collaborative of medical service organizations that serve uninsured and underserved populations.

Purpose: Improve access to care and quality of care available through medical safety net providers in Wake County.

How:
  1. Establish a web-based network for eligibility and screening of uninsured patients.
  2. Create a shared health record accessible to participating medical providers.
  3. Administer web based pharmaceutical patient assistance.
Outcomes:
  1. Improved identification and enrollment in reimbursable programs.
  2. Reduced duplication of services.
  3. Proactive management of complicated patients.
  4. Creation of a county-wide collaborative addressing the management of uninsured patients.
Cost/Term: $50,000 over two years.
