Purpose:
Improve access to care and quality of care available through medical safety net providers in Wake County.
How:
- Establish a web-based network for eligibility and screening of uninsured patients.
- Create a shared health record accessible to participating medical providers.
- Administer web based pharmaceutical patient assistance.
Outcomes:
- Improved identification and enrollment in reimbursable programs.
- Reduced duplication of services.
- Proactive management of complicated patients.
- Creation of a county-wide collaborative addressing the management of uninsured patients.
Cost/Term:
$50,000 over two years.
