ECHO (Education Community Health Outreach)

What: Family outreach and resource teams.

Purpose:
  1. Identify high risk, low performing children through referrals primarily from the southern and eastern regions of the Wake County school system.
  2. Assure access to health care and improve school success outcomes for children who are most at risk of failure in the public schools.
How: Provide comprehensive family-based intervention using home visits and family advocacy with families whose children are most at risk of poor health and school failure.

Outcome:
  1. Children and adults in participating families will be enrolled in health insurance.
  2. Children and adults identified to need services as a result of screenings will be referred to health providers and community resources.
  3. Children will be screened and assessed for special needs and referred to services, including Early Intervention.
  4. Children will experience educational success as evidenced by either school readiness level, growth in Literacy and Math assessment, and by End of Grade test results.
  5. Participating parents will acquire and demonstrate nurturing and support skills to promote optimal health and educational success for themselves and their children.
Cost/Term: $235,000 annually for a three year period.
