Pediatric Diabetes Program

What: A comprehensive pediatric diabetes program targeting low-income patients 5 - 18 to provide outpatient and inpatient diabetes case management; coordinated by a registered nurse/certified diabetic educator who will work with other nurses and dieticians, physician offices and the public schools.

Purpose: Reduce the medical and social complications of diabetes by providing comprehensive diabetes education to patients and their families.

How:
  1. Identify and recruit pediatric diabetes patients and families.
  2. Educate, over a two-year period, newly diagnosed patients and their families on the nature and management of diabetes.
  3. Evaluate the impact of the program in reducing the medical and social complications of diabetes.
  4. Collaborate with Wake County school nurses to implement diabetes management strategies.
Outcome:
  1. Decrease the number of emergency department and clinic visits.
  2. Decrease hemoglobin/blood sugar levels.
  3. Decrease the number of diabetic ketoacidosis admissions.
  4. Decrease the number of missed school days.
Cost/Term: $1,024,277 for a three-year period.
