Pediatric Diabetes Program Expansion

Purpose:
- Enhance interventions for children with pre-diabetes and diabetes with direct physician care services.
- Expand the Pediatric Diabetes Program (PDP) education/exercise program for at-risk children.
- Develop quality research data to measure impact and support advocacy.
How:
- Provide direct physician medical care from PDP staff physician.
- Use a full time Registered Nurse to support PDP physician and work with local partners to develop obesity programming.
- Underwrite expenses for expanded exercise intervention program to reach younger children through additional community sites.
- Conduct ongoing program evaluation and research activity in collaboration with area researchers in the UNC system.
Outcomes:
- 28 to 30 patients per week seen in clinic.
- Expansion of Energize program for 6 to 10 year olds and continued growth for 10 to 18 year olds.
- 500 referrals to PDP program annually - 150 enrollments into Energize.
- Conference to present lessons learned from program.
- Expansion of database with medical measurements and evaluations of program enrollees.
Cost/Term:
$614,822 over three years.
