Children's Access Program
What:
Improve enrollment, access, and outcomes for Wake County children.

Purpose:
Increase enrollment of Wake County children age birth through nineteen eligible for Medicaid and NC Health Choice and to assist enrolled children in accessing public and private practices accepting these insurances.
How:
- Building awareness of the importance of continuous enrollment through media.
- Placing case managers in childcare centers, targeted schools, targeted pediatric practices, emergency departments.
- Assuring that enrolled children follow EPSDT guidelines.
- Improving content of preventive health visits to assure identification and referral of conditions and to improve follow-up for conditions identified.
- Identifying children seen in emergency departments for non-emergent conditions and counseling parents on appropriate use.
- Identifying asthma and diabetes patients, reviewing records of care and incorporating best practice protocols.
Outcome:
- Enrollment goal year 1: increase participation in Medicaid and Health Choice by 2,300.
- Reenrollment goal year 1: 1,850 enrollees (60%).
- Health Check goal year 1: increase aggregate compliance to 64%.
Cost/Term: $1,290,252 over three years.
