Heritage Park Project

Program to integrate family, health and education services to neighborhood families.

Purpose:
Create a public housing site program to increase the access of resident children to needed health care that can be sustained by parents who have received training and support regarding enrollment and renewal of health insurance and well visit participation.
How:
- Screen pre-K through high school children and parents.
- Conduct outreach for health insurance enrollment.
- Develop parental responsibility for children's health care.
- Provide health education workshops.
- Conduct follow up efforts to assist with insurance renewal.
Outcome:
- Increase number of Heritage Park children enrolled in Health Choice and Health Check.
- Assist each family with insurance renewal process.
- Screen parents and children for common medical conditions.
- See that appropriate referrals are made and appointments are kept.
- Involve parents in family strengthening programs.
- Improve academic performance.
Cost/Term:
$70,000 over three years.
