Community Health Center Case Management Program

Purpose:
Increase enrollment of Wake County children age birth through nineteen eligible for Medicaid and NC Health Choice and to assist enrolled children in accessing public and private practices accepting these insurances.
How:
- Enrolling on site and screening 100% of families with three case managers.
- Using case workers to initiate well-child appointments, reschedule when necessary and reenroll as well as to do community outreach with faith organizations and community groups in selected areas of the county.
Outcome:
- 10% annual increase in number of insured children in practice on a base of 4800.
- 90% compliance with preventive screening schedule.
- Semiannual surveys to monitor progress and identify reasons for disenrollment.

$465,000 over three years. November 2001-November 2004
