Children's Health and Development Program (CHDP) – Phase II

Purpose:
Children in foster care have higher rates of social, emotional and behavioral problems, chronic physical disabilities and developmental delays. Child welfare agency policies are not current with recognized standards of care.
- Continue development of CHDP started with Rex Endowment funds in 2003.
- Create comprehensive assessment and management program for all children entering foster care in Wake County.
How:
- Maintain half-time Developmental Specialist;
- Add 25% to make Assessment Coordinator a full time employee;
- Complete data entry for CHDP records into foster care tracking system.
Outcome:
- Medical, developmental, and mental health needs of children in foster care will be identified and services will be secured;
- Individualized plans of care will be written and shared with DSS, families and medical home;
- Feasibility of creating two additional regional sites to serve foster families will be examined;
- Revenue sources to support Developmental Specialist will be identified.
Cost/Term:
$168,388 over three years. September 2007 - August 2010
