Purpose: Improve and expand CHDP services to refine current procedures and processes, add dental services, and develop procedures and processes of children upon reunification with biological parents.
How:
1. Continue coordination of a care plan for all foster care children.
2. Develop and implement a new dental health component within existing program structure.
3. Create position and determine roles, procedures, and processes for a family liaison to provide in-home service with families and engage and offer guidance to biological parents.
Outcome:
1. WCHS will sustain funding for Family Liaison position following the grant term.
2. Increased appropriate and timely case closures.
3. Increased Children and Family Services Reviews scores.
4. Improved efficient use of Medicaid funding.
5. Improved overall oral health for children that have maltreatment issues.
6. Secured care and stabilized services for children that have maltreatment issues.
7. Implementation of integrated CHDP services.
8. Less time involved with DSS services.
9. Increased family functioning.
10. Increased ability to obtain and maintain appropriate services.
Cost/Term: $304,627 over three years.
