Purpose: Bridge gaps in health delivery system with two nurse practitioners and four community lay health advisors working with established contacts in seven underserved neighborhoods.
How:
- Strengthen working relationships with other community agencies to provide outreach and linkage to services.
- Work with team of social workers, school nurses, mental health counselors and others funded by other organizations.
- Provide health services and referrals for underserved residents in targeted neighborhoods.
Outcome:
- 1,000 children/adults will receive health screenings.
- 1,800 children/adults will receive information regarding health resources.
- 75% will receive health counseling on specific concerns.
- 65% will receive short- term medical treatment.
- 75% will be linked to a permanent health care provider.
- 20 to 30 lay health advisors will receive 40 hours of leadership training.
Cost/Term: $419,027 over two years. July 2005-June 2007
