ECHO (Education Community Health Outreach) - Phase I

Purpose:
- Identify high risk, low performing children through referrals primarily from the southern and eastern regions of the Wake County school system.
- Assure access to health care and improve school success outcomes for children who are most at risk of failure in the public schools.
How:
Provide comprehensive family-based intervention using home visits and family advocacy with families whose children are most at risk of poor health and school failure.
Outcome:
- Children and adults in participating families will be enrolled in health insurance.
- Children and adults identified to need services as a result of screenings will be referred to health providers and community resources.
- Children will be screened and assessed for special needs and referred to services, including Early Intervention.
- Children will experience educational success as evidenced by either school readiness level, growth in Literacy and Math assessment, and by End of Grade test results.
- Participating parents will acquire and demonstrate nurturing and support skills to promote optimal health and educational success for themselves and their children.
Cost/Term:
$235,000 annually for a three year period. January 2003-June 2005
