Need:
Domestic violence has negative impact on the development, behavior, education, health and mental health of children and youth. The emotional consequences of being injured, harassed, or terrified can lead to an abused parent being less attuned to their children's needs. Safety concerns serve as a barrier for abused parents and children to access health care services.
Purpose:
The Oberlin Outreach Center at the new Interact facility will provide basic primary care services to approximately 250 children and 100 adults, and act as a bridge to other Wake Health Services sites as a permanent medical home for both the parent and children.
- Availability of services: Services for in-shelter clients will be provided when the Interact facility opens two half days per week and after six months three days per week. Services will be offered to outpatient clients (both prior in-shelter clients and non-shelter clients) six months after the facility opens.
- Providers of services: A mid-level provider will provide basic primary care services to patients. A registered nurse will provide follow-up services and patient education.
- Types of services: Services will include well child visits, preventative screenings and family planning for mothers, tuberculosis monitoring, and acute care treatment.
- Other support: Medicaid eligibility assistance and medication assistance will be provided. All clients will transition to other Wake Health Services sites as their situation stabilizes. Electronic health records will facilitate the coordination of care.
- Remove safety concern that prevents clients from obtaining medical care.
- Decrease in emergency room visits referred from Interact.
- Measured impact as a bridge to a permanent medical home:
- Number of children enrolled in public insurance.
- Number of children bridged to a medical home.
- Number of children with a medical home 3 months after leaving shelter.
- Increased capacity to pursue other funding to support non-billable services.
Cost / Term: $122,444 over three years. $40,226 in Year 1.
