Líderes de Salud – Phase II

Purpose:
Recent Latino immigrants still adapting to a new language and a new system of care struggle to understand and access existing services while the existing safety net system struggles to provide appropriate care.
- Connect growing population of low income, underinsured residents to health services with assistance of trained lay health advisors.
How:
- Improve curriculum used to train lay health advisors based on evaluation of Líderes de Salud I;
- Strengthen promotores leadership training;
- Build partnerships with two community sites that serve as operational base for promotores;
- Develop collaboration between Líderes program, CapitalCare Network, and others to create community led dialogues.
- Increase Latinos' awareness of importance of care and paths to availability.
Outcome:
- Sixty to ninety promotores trained in improved curriculum;
- Promotores demonstrate knowledge of available providers and means to services;
- Partner sites will have improved linguistic and culturally appropriate services;
- Health information messages will be developed and disseminated;
- Three hundred families will receive health information through direct contact.
Cost/Term:
$352,988 over three years. September 2007 - December 2010
