Need:
For patients with acute conditions, lack of access to necessary testing and medications can result in misdiagnoses or no diagnoses and can lead to increased emergency room visits. For patients with chronic conditions, lack of access can negatively impact their health outcomes.

Purpose:
Provide needed medications and diagnostic lab testing to uninsured teens in order to decrease health disparities in high-risk youth.

How:

  1. Adolescents will have access to necessary care regardless of ability to pay.
  2. Uninsured patients will have access to same standard of care as insured patients - compliance with recommended treatment will increase.
  3. Sexually active patients will have access to contraceptive options and have a documented contraceptive plan.

Outcomes:

  1. 100% of uninsured teens have access to prescription medications and diagnostic labs.
  2. Increase in compliance with recommended treatment plans.
  3. Decrease in number of unnecessary follow-up visits.
  4. 100% sexually-active teens have access to affordable contraceptive options.
  5. 100% sexually-active teens receive education about abstinence and risks and responsibilities of sexual activity and have a documented contraceptive plan.

Cost / Term: $72,576 over two years.
