Purpose: Reduce the medical and social complications of diabetes by providing comprehensive diabetes education to patients and their families.
How:
- Identify and recruit pediatric diabetes patients and families.
- Educate, over a two-year period, newly diagnosed patients and their families on the nature and management of diabetes.
- Evaluate the impact of the program in reducing the medical and social complications of diabetes.
- Collaborate with Wake County school nurses to implement diabetes management strategies.
Outcome:
- Decrease the number of emergency department and clinic visits.
- Decrease hemoglobin/blood sugar levels.
- Decrease the number of diabetic ketoacidosis admissions.
- Decrease the number of missed school days.
Cost/Term: $1,024,277 for a three-year period. February 2003-January 2006
