Purpose: Secure donated physician care, hospital services and medication assistance for uninsured children with medical needs from families with income below 185% of federal poverty level.
How:
- Maintain a network of participating sites and volunteer physicians.
- Screen and enroll qualified children at primary care sites.
- Provide case management and referral for qualified children.
Outcome:
- Approximately 300 to 350 children will receive medical assistance.
- Drug assistance will be provided to approximately 30 children.
- Qualified children will receive $300,000 in donated care from physicians and hospitals.
Cost/Term: $50,000; January 2006-December 2007
