Strengthening a Medical Home for Youth

Purpose:
Teens presenting to Wake Teen often have several challenging health issues compounded by other social/life situation factors that interfere with effective care for health concerns.
- Strengthen Wake Teen's capacity to manage care for this at risk population by improving agency infrastructure.
How:
- Add one full time nurse;
- Develop tracking and referral protocols.
Outcome:
- Increase patient compliance and follow through with referrals to outside specialists;
- Increase capacity to serve more adolescents by improving office efficiency.
Cost/Term:
$118,932 over two years. September 2007 - August 2009
