The following forms should only be submitted if they are applicable to your application:
Partner Agreement Form
- For Capacity Building Initiative (CBI) or project grant applications: Form used when an application involves partner organizations and the work of the grant is highly dependent on their participation.
Fiscal Sponsor Signature Page
- For CBI grant applications: Form used when an application is being submitted by a collaborative that does not have 501(c)(3) status.
- For project grant applications: Form used when an application is being submitted by a collaborative or an organization that does not have 501(c)(3) status.

- For CBI grant applications: Form can be used to detail project activities.
